The Heart Manual is a home-based self-management program for people with coronary artery disease and related conditions. Patients are guided through the Heart Manual program by a trained Facilitator.

Through the Heart Manual program, Impact BC aims to increase availability of and access to cardiac rehabilitation services to patients.
